Medical staffing for inpatient care in a teaching hospital in Brazil. Part I. General aspects and preliminary outcomes.
This study presents the results of assessing medical staff staffing in an inpatient hospital of a university teaching hospital in Brazil. Part I of the study describes the background information of the hospital and describes the method of medical staff staffing. In part II we present the preliminary outcomes of the staffing. Data were collected through a questionnaire and by review of hospital charts. Part I: The study was performed in the inpatient hospital of a teaching hospital in Natal. The hospital has 215 beds, 404 physicians and 2,127 nurses, all working in seven medical floors, which include 73 specialties. The data collected through the questionnaire indicate that: 44.2% of medical staff (including residents) has less than two years of experience, and this percentage reaches 75% in more than 50% of medical floors; when comparing work activities that lead to most stress, the highest percentage of stress is reported by physicians (14%) and nurses (16%); the percentage of persons working a double shift (up to two shifts per day) is nearly 10%; and the percentage of persons involved in quality control of the service is 37%. Although the number of physicians and nurses, the percentages of physicians and nurses working a double shift, and the percentage of physicians involved in quality control of the service are relatively low in our study, the physicians’ stressfulness and the overall dissatisfaction of the medical staff seem to be higher than their Brazilian counterparts (11 and 6%, respectively). The average number of persons present in the medical staff at the time of observation was 45.8% (41% of all the physicians, 34% of all the nurses, and 55% of all medical residents).
