Short- and long-term safety of transfusion of pre-storage leukoreduced red blood cells.
Non leukoreduced blood components have been in use for more than three decades, but the concerns about the possible risk of the transmission of blood-borne infections have resulted in the development of a number of pre-storage leukoreduction and/or filtration techniques. The safety of stored pre-storage leukoreduced blood components is now well established. The objective of this study was to assess the safety of pre-storage leukoreduced red blood cells in blood bags for transfusion within 24 hours of collection. A retrospective cohort study was performed at the blood bank of the Royal Victoria Hospital in Belfast. Three consecutive cohorts of pre-storage leukoreduced red blood cell units, preserved using antifreeze storage at 4°C, were analysed for the short and long term safety of transfusion. From 2 December 2010 to 31 December 2011, a total of 391 blood bags were transfused; 72% were transfused within 24 hours of blood collection. In cohort 1, 101 blood bags (25%) were transfused within 24 hours, and no transfusion-transmitted infections were detected during hospital stay or at 3 months after transfusion. In cohort 2, 20 blood bags (13%) were transfused within 24 hours, and there were two hospital-acquired infections after 3 months (one bacterial septicaemia and one viral infection). In cohort 3, 122 blood bags (31%) were transfused within 24 hours, and three hospital-acquired infections after 3 months were detected. Blood component use remains common in ICU patients.
