To assess the extent of prescribing of fertility drugs (mainly clomiphene citrate and gonadotrophin treatment) in England and Wales and to analyse the effect on pregnancy rates. We analysed routine data on fertility drug use taken from laboratory returns for the calendar years 1994 to 1998. Where possible, data were extrapolated to 1998 and the impact on pregnancy rates estimated. In 1998, 8,184 women were prescribed clomiphene and 1,541, gonadotrophins (mainly follicle-stimulating hormone). The overall pregnancy rate in women prescribed clomiphene was 15.9%. Use of clomiphene citrate has risen over the last decade and the increased use of gonadotrophins in 1998 was accompanied by a significant increase in pregnancy rates in the two most productive age groups–35-39 and 40-44. Despite this, the overall rate remains at around half the rate in Europe. The major limitation of this analysis is the restriction of it to the prescribing of one type of drug, clomiphene. Furthermore, it is possible that the prescribing of clomiphene has been influenced by the National Institute for Clinical Excellence’s advice in mid-1998. Our review confirms the widespread use of clomiphene in the UK and that the rate of conception has increased since the NICE guidelines came into force. However, there is evidence that gonadotrophin use has increased and that this may limit the increased rate of conception.
