      To assess the effect of precalibration and base-line correction on the frequency of false-positive and false-negative results of the cut-point method for the detection of biliary tract malignancy. Twenty-six prospectively studied patients, whose computerized tomography (CT) scans identified a common bile duct lesion, were included in the study. All CT scans were obtained with a multi-detector CT scanner using both intravenous and oral contrast agent administration and contrast-enhanced cholangiography. Five readers interpreted 20 CT scans in two separate sessions, either following a precalibration protocol (without base-line correction) or following a precalibration protocol and base-line correction. In the precalibration protocol without base-line correction, readers identified a lesion in 17 (85%) of 20 patients, incorrectly identified as pathology in 3 (15%) of 20 patients, and overestimated true-positive lesions in 2 (10%) of 20 patients. In the precalibration protocol with base-line correction, readers identified a lesion in 18 (90%) of 20 patients, incorrectly identified as pathology in 1 (5%) of 20 patients, and overestimated true-positive lesions in 1 (5%) of 20 patients. Overall, a change in the interpretation of 9 (45%) of 20 patients’ CT images was seen, compared with a change in 10 (50%) of 20 patients’ CT images when base-line correction was used. The sensitivity of the cut-point method for identification of biliary malignancy was 95% with base-line correction and 85% without base-line correction. Base-line correction had a minimal effect on the specificity (75% vs. 80%) but was associated with a higher rate of overdiagnosis (20% vs. 10%). Precalibration and base-line correction have a minimal effect on the sensitivity of the cut-point method for the detection of biliary tract
