To assess the accuracy of magnetic resonance (MR) imaging in predicting the optimal position for percutaneous nephrolithotomy (PCNL) and to evaluate which MR imaging characteristics are associated with the appropriate placement of a renal puncture. This retrospective study involved 119 patients with stones sized from 6 to 30 mm who underwent MR imaging before PCNL. The data for the patients were analyzed with respect to the degree of hydronephrosis, position of the kidney, stone location in relation to the renal pelvis and sinus, and stone size. The correct puncture points for PCNL were determined using three-dimensional computed tomography (3DCT) and were compared with the renal positions predicted by MR imaging. The stone locations in relation to the renal pelvis and sinus were classified as medial, central, or lateral. The stone sizes were classified as being larger than 7 mm or smaller than 7 mm. The overall accuracy of MR imaging in predicting the optimal puncture point for PCNL was 93.5%. The average absolute difference in the correct puncture site measured with 3DCT from the predicted site by MR imaging was 3.3 mm, and the overall relative error in the renal location was 6.2%.
