Six of these strokes were cerebral infarctions and were attributed to isolated internal carotid artery occlusion or stenosis. In 18 perioperative cerebral embolic strokes, the internal carotid artery was stenosed in three. Non-fatal ischemic strokes occurred in 27 (32%) and were attributed to intracranial arterial stenosis in 17 cases. These strokes, were symptomatic in only 3 patients. One non-fatal stroke resulted in a major neurological deficit. Carotid endarterectomy was performed for symptomatic stenosis of the internal carotid artery. In general, the stenosis was severe and the surgical risk was high. The cumulative stroke rate in 10 years was as high as 43% (95% confidence interval, 29-59%).
